The Health office personnel provides assistance with first aid, administered medication to students and notify the District Nurse, parents and or school principal in the event of health problems or injuries at school. They also assist with vision and hearing screening and other special health needs.

Illinois law requires that school districts have on file medical verification of certain immunizations for students entering school. Immunization dates and records must be submitted on the Child Health Form, signed, stamped, and dated by the health care provider.

Varicella immunization on or after the first birthday, or a doctor’s verification of having had the disease is required for all children entering a child care facility, a preschool program or Kindergarten- 8th grade.
Three doses of the Hepatitis B vaccine are required for all students in childcare and preschool as well as those in 5th through 8th grade. The interval between the first two doses must be no less than four weeks, the interval between the second and third dose must be at least two months, and the interval between the first and third dose must be at least four months.
